Waiver of Liability, Release, Assumption of Risk & Indemnity Agreement Notice. This is a legally binding agreement. I understand that by signing this On-Site Childcare Waiver of Liability, I release and hold harmless Children Event Sitters, LLC, and its owners, directors, officers, advisors, employees, agents, instructors, volunteers, childcare workers, and all other persons or entities acting for them, from any and all claims, demands, suits, costs, and charges in connection with or arising out of Children Event Sitters, LLC childcare services. This includes, but is not limited to, personal injury, bodily harm, or property damage occurring while the above child/children is/are in their care at Children Event Sitters, LLC. I understand that I must pay the set childcare fees. Fifteen (15) minutes or more will be charged as a one-hour rate. I understand that if my child becomes inconsolable during the class session, I am responsible for leaving the event to attend to my child. I am responsible for feeding my child and ensuring they use the restroom prior to the start of the event, to the best of my ability. Food and drink must be provided by parents. Infant supplies (such as diapers and wipes) must also be provided by parents. I understand I must remain at the hosted event location at all times. I have read and understand the Children Event Sitters, LLC On-Site Childcare Policy. Additional Notice: I further acknowledge and agree that Children Event Sitters, LLC shall not be held liable or responsible for any injuries, accidents, or damages of any kind that may occur in the playroom area during open play sessions or drop-off care. Participation in these activities is voluntary and at the parent’s and child’s own risk. By signing below, I acknowledge that I have carefully read, fully understand, and voluntarily agree to this waiver and its terms.
